What is a health and wellness coach?

A health and wellness coach is a behavior change expert who guides and supports individuals toward making positive lifestyle changes related to their personalized health and wellness goals.  I am passionate that movement and eating choices can impact all areas of our lives and my expertise in physical movement and functional nutrition allow me to help guide you in the best way possible.  I would love to work with you in determining the best plan for you and your individual needs and desires.

Do I need a doctor's referral before setting up a visit?

Accepted Answer

No you do not!  Illinois is a direct access state for physical therapy, allowing for physical therapy evaluations and treatment without a physician referral.  Occasionally your health insurance may require a referral for reimbursement purposes.  
There is no referral needed for health and wellness coaching.

Do you take my health insurance plan?

Accepted Answer

Health and wellness coaching is not yet covered by individual health insurance, but stay tuned because hopefully it will be coming soon.

All physical therapy and health and wellness coaching sessions are payable through Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) as well as with cash (including Venmo and Zelle) or credit cards.

I am an in-network provider and am covered by most Blue Cross Blue Sheild PPO and UnitedHealthcare PPO plans for physical therapy in Illinois.  I am considered out-of-network for other insurance plans, but I can provide a super bill to submit for reimbursement.

What are my credentials?

I am a licensed physical therapist with a Doctorate in Physical Therapy from Temple University and over 20 years of experience. I am a Certified Health and Wellness Coach as well as a Certified Functional Nutrition Expert from MindBodyGreen.

I am happy to work with you to create a plan that best suits you and your unique desires. Please reach out and we can figure it out together!
